Sobe Innovative Rehab in Clearwater, FL is seeking a licensed Occupational Therapist to provide outpatient therapy services in a home setting. Join our team to help older adults maintain their independence and quality of life through compassionate, evidence/comfort-based care.

Responsibilities include evaluating and treating residents at home, developing and implementing individualized treatment plans, collaborating with physicians and families, and maintaining accurate EMR documentation.

Qualifications

• Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program.
• Active Florida Occupational Therapist license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in a home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
