Job summary

Ambercare Hospice in Las Vegas, NM seeks a compassionate PRN Nurse Practitioner to provide expert clinical care to patients and families at home. You will assess eligibility, coordinate with the inter-disciplinary team, and deliver high-quality hospice care in a home setting.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a Nurse Practitioner in the state of practice.
• Minimum of two years of varied work experience as a professional nurse.
• Must possess current CPR Certification.
• Demonstrated skill in nursing practices and clinical judgments.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to cope with family emotional stress and tolerance of individual lifestyles.
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Determine patient eligibility for care based on State and Federal guidelines.
• Collect data on physical, psychological, social, and spiritual factors and initiate intervention with the interdisciplinary team.
• Coordinate admission or recertification for care with physician and other agencies.
• Utilize resources to meet patient needs and update the plan of care.
• Provide regular home visits to reassess eligibility and needs.
