Job summary

IntraCare is seeking a PRN Advanced Practice Provider (APP) to deliver high-quality primary care services across its clinics in Texas and Arizona. This role is ideal for an experienced Nurse Practitioner (NP) or Physician Assistant (PA) looking for flexible, as-needed work.

You will be responsible for comprehensive patient evaluations, diagnosing and managing acute and chronic conditions, developing treatment plans, prescribing medications, and ordering/interpreting diagnostic tests.

Qualifications

Current and unrestricted NP or PA license in the applicable state.
Graduate degree from an accredited NP or PA program.
National board certification appropriate to the provider's discipline.
Current BLS certification.
Eligible for or able to obtain applicable state prescribing privileges.
Ability to work independently while collaborating effectively with the healthcare team.
Experience with electronic medical records.

Responsibilities

Conduct comprehensive patient evaluations, medical histories, and physical examinations.
Diagnose and manage acute and chronic medical conditions within scope of practice.
Develop individualized treatment plans and provide appropriate follow-up care.
Prescribe medications and treatments in accordance with applicable laws and clinical guidelines.
Order, interpret, and follow up on laboratory tests, diagnostic studies, and imaging.
Provide preventive care, health education, and counseling to patients.
Manage chronic disease conditions and monitor treatment effectiveness.
Identify changes in patient condition and determine appropriate escalation or referral.
Collaborate with physicians and other healthcare professionals regarding patient care.
Maintain accurate, timely, and complete documentation in the electronic medical record (EMR).
Complete clinical documentation, orders, referrals, and other required records within established timelines.
Provide coverage for clinics based on PRN staffing needs and provider availability.
Follow all organizational policies, clinical protocols, compliance requirements, and quality standards.
Participate in quality improvement and patient safety initiatives as requested.
Communicate effectively with patients, families, clinical staff, and leadership.
Maintain patient confidentiality in accordance with HIPAA and applicable regulations.
Perform other clinical duties as assigned.
