Job summary

Qualifications

• Graduate of an approved/accredited college program meeting CCC-SLP requirements.
• Active Florida SLP license.
• Minimum 1 year of experience as a licensed SLP; home health experience preferred.

Responsibilities

• Perform initial and ongoing assessments, including OASIS assessments.
• Develop a plan of care in consultation with the physician and care team.
• Instruct patients and families on swallowing safety and communication strategies.
• Prepare clinical and progress notes of services provided and submit timely.
