Job summary

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities across Fairfax County, including Fairfax, Centreville, Chantilly, Reston, Herndon, Vienna, McLean, Springfield, Burke, Annandale, Lorton, Oakton, Great Falls, Clifton, Falls Church. Qualifications

• Active Virginia Speech-Language Pathologist License.
• Graduate Degree in Speech-Language Pathology from an accredited program.
• Current ASHA Certification (CCC-SLP) Preferred.
• Valid Driver's License and reliable transportation.
• WellSky Experience is a Plus.

Responsibilities

• Complete speech-language pathology evaluations in patients' homes
• Develop individualized treatment plans
• Treat speech, language, communication, cognitive, and swallowing disorders
• Provide dysphagia treatment and swallowing education as appropriate
• Help patients improve functional communication and cognitive skills
• Educate patients, families, and caregivers
• Monitor progress and adjust treatment plans as needed
• Coordinate care with the interdisciplinary team
• Complete reassessments and discharge planning
• Maintain accurate and timely documentation using WellSky

Skills

Virginia SLP license

CCC-SLP certification

Strong communication skills

Ability to work independently

Home health experience

Education
