Job summary

Empath Health’s Empath Home Health in Florida is seeking a compassionate PRN Home Health RN to deliver skilled nursing care in patients’ homes across north Hillsborough County, including Lutz, New Tampa, Tampa Palms, Temple Terrace, and University areas.

Requirements include an active Florida RN license and 1 year of direct patient care; BSN/MSN preferred. Weekend/holiday on-call rotations are required, with potential to expand to full-time as the market grows.

Qualifications

• Active Florida RN license required; BSN or MSN preferred.
• Minimum 1 year direct patient care experience; home health strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
