Job summary

Klarus Home Health in Granbury, TX is seeking a PRN licensed physical therapy assistant to provide coordinated PT services to home health patients under the supervision of a Licensed Physical Therapist. This role offers autonomy and one-to-one patient care in a home-based setting.

Requirements include an Associate Degree, current state license, CPR, and at least one year of PTA experience preferred, with emphasis on home health and HCHB EMR familiarity.

Qualifications

• Graduate of an accredited PTA program with an Associate Degree.
• Current State License or PTLC-compatible license where applicable.
• One year PTA experience preferred, home health experience strongly preferred.
• HCHB EMR system knowledge preferred.
• CPR certification required.
• Ability to coordinate care and communicate effectively with patients and families.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide coordinated physical therapy to home health patients under PT supervision.
• Develop and implement individualized PT plans and goals.
• Monitor patient progress and adjust treatments as needed.
• Collaborate with caregivers and other health professionals to coordinate care.
