Renew Home Health is seeking a compassionate Physical Therapist to provide in-home therapy across the Abilene area in a PRN capacity. You’ll evaluate patients, craft individualized plans, and guide recovery in a one-on-one setting, right at the patient’s home.

Qualifications

• Active Texas PT license in good standing.
• Home health experience preferred.
• A valid Texas driver’s license and reliable transportation.

Responsabilités

• Evaluate patients and develop individualized therapy plans of care.
• Provide skilled physical therapy visits in patients’ homes throughout the Abilene area.
• Help patients improve strength, mobility, balance, and independence.
• Adjust the plan of care as patients progress and collaborate with the care team.
• Teach patients and families home exercises and safety strategies.
