Job summary

AdventHealth Primary Care Network seeks an Advanced Practice Provider to join our team in Florida. You will deliver comprehensive primary care across the lifespan, emphasizing preventive care, chronic disease management, and patient-centered outcomes.

Qualifications

• Experience delivering primary care to diverse age groups.
• Ability to manage acute and chronic conditions across the lifespan.
• Skill in coordinating referrals and tests with a care team.

Responsibilities

• Provide comprehensive, patient-centered primary care from pediatric to geriatric; including women's health.
• Deliver independent and collaborative medical management of acute and chronic conditions.
• Order and interpret diagnostic testing, including labs and imaging.
• Monitor treatment plans and coordinate referrals to physicians or specialists.
• Collaborate with physicians and staff to sustain continuity of care.
• Deliver care with professionalism, compassion, and patient focus.
