Job summary

Crouse Community Center in Morrisville, NC is seeking a Per Diem Physical Therapist to join the rehabilitation team. You will provide high-quality PT services to residents, focusing on mobility, independence, and well-being in a long-term care setting.

The role emphasizes flexibility with per diem scheduling, collaboration with nursing and interdisciplinary teams, thorough documentation, and adherence to infection control and facility policies.

Qualifications

• Current NYS Physical Therapy license in good standing.
• Bachelor’s or Doctorate in Physical Therapy from an accredited program.
• Experience in long-term care or similar healthcare setting preferred.
• Strong communication skills and a commitment to high-quality resident care.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Professional, positive, and resident-focused demeanor.

Responsibilities

• Deliver resident-centered PT services including evaluation and treatment planning.
• Document assessments and progress in accordance with regulatory and facility standards.
• Collaborate with nursing, medical staff, and other disciplines in care planning.
• Ensure safe, effective use of therapy equipment and maintain a clean environment.
• Support MDS and interdisciplinary processes through timely communication.
