Job summary

Yale New Haven Health is seeking a PA or NP for the Northeast Medical Group Internal Medicine practice in Hamden, CT. This full-time role is per-diem as needed and requires CT licensure and relevant certification.

You will work with a multidisciplinary team focused on comprehensive primary care and patient-centered outcomes. Responsibilities include health assessments, care planning, medication management, and patient education.

Qualifications

• 1-2 years clinical experience in a primary care/internal medicine setting.
• Active CT medical license or in process of obtaining.
• ACLS/BLS/PALS certifications as required.
• DEA license.

Responsibilities

• Conduct comprehensive health assessments and obtain detailed medical histories.
• Develop and implement individualized treatment plans with supervising physician.
• Prescribe medications, order therapies, and manage medical devices per scope.
• Provide patient education on health maintenance, disease management and adherence.
• Evaluate and manage common acute illnesses and chronic conditions.
• Coordinate referrals and follow-up care with the healthcare team.
• Document encounters in the electronic health record and participate in quality initiatives.
