Qualifications

• Current Hawaii Physical Therapist (PT) or Physical Therapist Assistant (PTA) license required.
• Home health experience preferred.

• Ability to work independently and manage a flexible schedule.

Responsibilities

• Perform skilled home health visits in accordance with physician orders and care plans.
• Conduct physical therapy evaluations, reassessments, and treatment interventions.
• Implement and monitor therapeutic exercise and mobility programs.
• Provide fall prevention assessments and recommendations.
• Educate patients and caregivers on safety, mobility, and home exercise programs.
• Collaborate with physicians, nurses, and interdisciplinary team members.
• Complete accurate and timely Medicare-compliant documentation.
