Qualifications

• RN license or MSW or equivalent credential is required.
• 3+ years of experience in geriatric care management, aging services, home care, or case management is required.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Conduct in-home client assessments and build personalized care plans that support health, independence, and quality of life.
• Connect clients with healthcare services, community resources, and support programs.
• Act as the client's and family's advocate in healthcare decisions.
• Coordinate with physicians, providers, caregivers, and community partners so nothing falls through the cracks.
• Support clients through transitions such as hospital discharge and placement in rehab, assisted living, or skilled nursing.
• Monitor progress and adjust care plans as a client's needs change.
