Job summary

Wyomissing Health & Rehabilitation Center in Reading, PA, is seeking a Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team. You will evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders and develop individualized treatment plans to maximize outcomes.

Qualifications

• Current Pennsylvania Speech-Language Pathologist license.
• CCC-SLP certification required.
• Master's degree in Speech-Language Pathology from an accredited program.
• SNF/Long-Term Care experience preferred.
• Experience with Net Health and PCC is a plus.
• Strong clinical assessment, communication, and organizational skills.
• Passion for providing compassionate, resident-centered care.

Responsibilities

• Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.
• Develop and implement individualized treatment plans to maximize functional outcomes.
• Provide skilled speech therapy services to short-term rehab and long-term care residents.
• Perform dysphagia assessments and implement swallowing interventions.
• Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary team.
• Educate residents, families, and caregivers regarding treatment plans and communication strategies.
• Complete accurate documentation using Net Health and PCC.
• Maintain compliance with all federal, state, and facility regulations.
