Qualifications

• Physician from any medical specialty.
• Licensed or eligible for medical licensure in Oklahoma.
• Comfortable managing medically complex patients.
• Strong clinical judgment and decision-making abilities.
• Excellent communication and interpersonal skills.
• Able to work independently and collaborate with a multidisciplinary team.
• Organized and committed to timely documentation.

Responsibilities

• Provide medical care in skilled nursing facilities, rehabilitation centers, and other post-acute settings.
• Complete initial assessments, routine follow-up visits, and acute visits as needed.
• Manage chronic conditions and changes in patients’ health.
• Perform medication reconciliation and develop individualized care plans following hospital discharge.
• Collaborate with nurses, therapists, case managers, specialists, and facility staff.
• Communicate treatment plans and goals of care with patients and their families.
• Maintain accurate and timely electronic health record documentation.
• Support efforts to reduce hospital readmissions and improve patient outcomes.
