Qualifications

• Must have a valid Ohio driver's license and proof of insurance.
• 6 months experience in personal care is preferred, but not required.
• Current CPR/First Aid certification.

Responsibilities

• Provide community inclusion and personal care services.
• Assist with daily living activities and health equipment.
• Deliver timely reporting of service plan deliverables.
