Job summary

Arista Home Care Solutions in Toledo, Ohio, is seeking a dependable Licensed Practical Nurse (LPN) for part‑time, per‑visit home health care.

The position offers $25.00 per visit (40–45 minutes) with additional holiday pay, travel time reimbursement ($12.50/hr), mileage reimbursement, paid training, and 24/7 on‑call clinical manager support. Flexible scheduling is provided to fit patient needs and nurse availability.

Qualifications

• Graduate of a state‑approved practical nursing program.
• Current, valid Ohio LPN license in good standing.
• Available for part‑time, per‑visit work, including rotating weekends with other LPNs.
• Current CPR certification; valid driver's license; current auto insurance; and reliable access to a vehicle.
• Successful completion of required pre‑employment screening, including BCI background check, BMV check, Ohio Board of Nursing license verification, and drug screen.
• Basic computer skills and ability to complete electronic clinical documentation.

Responsibilities

• Provide skilled nursing services under the direction of a registered nurse and in accordance with authorized orders and the established plan of care.
• Observe and document the patient's condition; implement ordered nursing interventions; and contribute to the planning, implementation, and evaluation of care.
• Administer medications and treatments and provide other ordered skilled care within the LPN's scope, training, and demonstrated competency.
• Provide or reinforce patient and family education and communicate promptly with the RN, physician, and other members of the care team.
• Complete accurate, timely documentation and follow agency emergency procedures.
