Professional Case Management is hiring Home Health Aides/Certified Nursing Assistants to provide in-home care in Tennessee. You will create your own schedule and build one-on-one relationships with patients in their homes, delivering essential daily living support and compassionate care.

Ideal candidates are CNAs with active Tennessee certification and CPR, ready to document care accurately and work as part of a dedicated team.

Qualifications

• Active CNA certification in TN
• Current CPR certification
• TB test
• Experience in home health care preferred (training available)

Responsibilities

• Provide one-on-one in-home care to patients
• Assist with activities of daily living and personal hygiene
• Assist with light housekeeping and meal preparation
• Document patient status accurately and timely
• Coordinate with care team to ensure quality care
