Job summary

Seven Hills Foundation seeks a Case Manager I - Family Support in Athol, MA. The role coordinates Family Services activities, maintains contact with families, and works with DDS to support individuals with disabilities. Home visits and plan development are central to this position.

Responsibilities include intake processing, resource identification, and training families in utilizing supports. A valid driver’s license is required and the role emphasizes collaboration and community linkage.

Qualifications

• Minimum of three (3) years of advocacy experience working with children and adults with ID, autism, or developmental disabilities or related experience in a program setting.
• Valid Driver’s License is required.
• Bachelor degree preferred.
• 3 – 5 years of related experience

Responsibilities

• With the Area Director
• Coordinate Family Services intake procedures including the completion of all required forms, documentation, and Family Support Plan.
• Identify resources available to families and ensure supports are provided as required in the Family Support Plan.
• Provide training to families on the effective use of resources available for counseling, community and educational connections.
• Provide training to families geared towards the development of problem-solving skills and strategies.
• Create opportunities for families to support each other.
• Create skill development practice groups for personal safety, boundaries, social skills and prevocational training, job and peer development.
• Identify funding sources for individuals/families supported and monitor related expenditures.
• Manage the funding contractual agreement.
• Attend specified job-related trainings for professional development.
• Maintain connections with school programs, work and social programs.
• Return correspondence within twenty-four hours.
• Collaborate with program managers to share resources and ensure timely service delivery.
• Participate as a Family Services representative to committees/councils.
• Utilize community resources to enhance quality of life of families/individuals.
