Qualifications

• A minimum age of twenty-five (25) years.
• Possession of a valid driver’s license issued by your state of residence for the type of vehicle to be operated.
• Must be current on personal auto insurance policy.
• Must have an active cell phone plan, and provide proof of 6 consecutive timely payments if on prepaid.
• US citizen or resident alien with a TIN (SSN, EIN, ITIN acceptable).

Responsibilities

• Transport injured workers to appointments from your own vehicle, with rides scheduled in advance to fit your schedule.
• Get paid by the hour to wait at the facility when wait time is approved.
• Maintain safe, courteous transportation and cooperate with scheduling and facility staff.
