Job summary

CVHCare is seeking a Certified Home Health Aide (CHHA) to join our home health team in San Ramon, CA. You will work under the supervision of a Licensed Clinician, providing skilled services and assisting with daily activities for patients in the San Ramon area.

The candidate should have at least one year of CHHA experience, be capable of checking vitals and documenting in clinical charts, and hold CA CHHA certification with CPR certification.

Qualifications

• Minimum one year experience as CHHA.
• Minimum one year checking vitals and documenting on clinical charts.
• CHHA Certification and CPR Certification (CA).
• Home Health Aide Training is required.

Responsibilities

• Provide skilled services under supervision of a Licensed/Certified Clinician.
• Assist with activities of daily living and adjunct routines as prescribed by clinicians.
• Check vitals (temperature, pulse and respiration) and support prescribed exercises.
• Maintain thorough and timely chart documentation and data entry.
