Job summary

MedCap Health is seeking a compassionate Nurse Practitioner for post-acute and long-term care. The role supports a flexible schedule with full-time, part-time, and flexible options, on a W2 basis.

You will work with physicians to deliver high-quality care across facilities, focusing on both acute and chronic conditions. Enjoy malpractice coverage and administrative support, with no overnight or weekend shifts.

Qualifications

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
• 1+ years of experience in post-acute/SNF/LTC.
• Prior post-acute, long-term, and/or skilled nursing facility experience.

Responsibilities

• Provide high-quality care in collaboration with the attending physician.
• Manage patients with acute and chronic conditions in post-acute and long-term care settings.
• Collaborate with the multidisciplinary team including the Director of Nursing, Social Workers and Administrators.
