New grads welcome; core duties include conducted assessments with no treatment plans, submitting forms, and enjoying malpractice insurance coverage as part of the contract.

Qualifications

• Active, unrestricted state NP license.
• Board certification: FNP, AGNP, or ANP.
• Reliable personal transportation.
• Available for at least 60 hours per month.
• New grads welcome — no minimum experience required.

Responsibilities

• Perform pre-scheduled, in-home health assessments for Medicare Advantage, Medicaid, and commercial plans.
• Use pre-populated forms and structured workflows; no treatment plans.
• Complete the visit and submit the form; no follow-up calls required.
• Visits span multiple counties within your region; higher coverage increases earning potential.
