Job summary

Emporia Rehabilitation And Healthcare Center in Emporia, VA seeks a Speech Therapist to evaluate and treat residents in a long-term care setting. The role requires ASHA-approved credentials with state licensure and CFY completion, plus ability to work closely with the interdisciplinary team.

You will conduct evaluations, develop and document treatment plans, obtain MD orders for care, and maintain progress notes and discharge documentation.

Qualifications

• Must hold MA or MS in communication disorders recognized by ASHA and state board.
• Must hold current state license/registration as applicable.
• Successful completion of CFY as part of required credentialing.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying treatment as indicated.
• Document patient progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format promptly.
