Qualifications

• Active LPN license required.
• At least 1 year in skilled nursing.
• Reliable transportation required.
• Able to lift at least 50 pounds.
• Legally authorized to work in the United States.
• Willing to complete background and drug screen.
• Excellent attendance and punctuality.

Responsibilities

• Record patient information and vital signs in EMR.
• Administer medications per orders and policies.
• Monitor resident conditions and report changes.
• Document medications, treatments, and observations.
• Collaborate with CNAs and care team.
• Maintain infection control and regulatory compliance.
• Assist with assessments and care plans.
• Communicate concerns to supervisor.
• Perform other duties as assigned.
