Qualifications

• Active LPN license in Michigan (or Compact)
• Minimum 1 year in skilled nursing settings
• Reliable transportation to facilities
• Ability to lift up to 50 lbs
• Legal authorization to work in the United States
• Willingness to undergo background and drug screening

Responsibilities

• Record patient information and vital signs in EMR PCC
• Administer medications and treatments per orders
• Monitor residents and report changes to clinical team
• Document medications, treatments, and observations
• Provide direct nursing care including wound care
• Assist with assessments and care plan implementation
• Collaborate with CNAs and other staff to ensure quality care
• Maintain policy compliance and infection control standards
• Communicate concerns to supervisor and agency
• Perform other duties as assigned
