Qualifications

• Minimum 1 year of experience working under an active LPN license.
• Minimum 1 year of experience working in skilled nursing.
• Must have reliable transportation to assigned facilities.
• Must maintain consistent attendance and punctuality.
• Able to push, pull, and lift at least 50 pounds.
• Able to stand and walk with frequent bending, reaching, and lifting with proper body mechanics.
• Must be legally authorized to work in the United States.
• Willing to complete background and drug screening per applicable laws

Responsibilities

• Utilize EMR Matrix to record patients’ medical information and vital signs.
• Administer medications and treatments per physician orders and facility policies.
• Monitor resident conditions and report significant changes to the clinical team.
• Record and maintain accurate resident health information and vital signs.
• Provide direct nursing care including wound care within scope of practice.
• Assist with resident assessments and individualized care plans.
• Document medications, treatments and observations accurately.
• Collaborate with CNAs and other healthcare staff to deliver quality care.
• Maintain compliance with facility policies, infection control, and regulations.
• Communicate concerns to onsite supervisor and agency.
