Qualifications
- Active LPN license required in Tennessee.
- Minimum 1 year in long-term care setting.
- Reliable transportation to assigned facilities.
- Ability to lift up to 50 pounds.
- Willingness to pass background and drug screening.
Responsibilities
- Record patients’ medical information and vital signs in EMR Reliasto.
- Administer medications per physician orders and facility policies.
- Monitor resident health and report changes promptly.
- Provide direct nursing care within scope of practice.
- Assist with assessments and care plan implementation.
- Document medications, treatments, and observations accurately.
- Collaborate with CNAs, physicians, and other healthcare team members.
- Communicate with residents, families, and providers about needs.
