InHome Therapy is seeking a licensed Occupational Therapist to serve adult and geriatric patients in their homes around Rockford, IL. You will work a flexible schedule with a steady caseload and support from a focused team.

Responsibilities include delivering in-home OT services, assessing mobility and function, implementing rehab programs, educating patients and families, and maintaining Medicare-compliant documentation; a reliable vehicle is required.

Qualifications

• Active OT license in good standing.
• BLS CPR certification.
• Reliable transportation to visit patients in homes.
• Comfort with technology and EMR systems.

Responsibilities

• Deliver in-home occupational therapy services to adults and geriatric patients in the community.
• Evaluate mobility, strength and function to develop individualized goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain Medicare-compliant documentation and agency standards.
