Job summary

Northeast Healthcare Recruitment, Inc. seeks Nurse Practitioners with 1+ years of experience to conduct in-home Health Risk Assessments for Medicare Advantage patients. The role is a 1099 Independent Contractor arrangement and offers flexible hours, with work available 7 days a week between 8am and 8pm, plus mileage reimbursement and a $25 no-show payment.

Location includes Toledo, OH and surrounding counties. Each assessment lasts 45 minutes to 1 hour.

Qualifications

• Board-certified Nurse Practitioner licensed in Ohio.
• Home Health experience.
• Must be able to cover Lucas and Ottawa counties.

Responsibilities

• Conduct comprehensive Health Risk Assessments for Medicare Advantage patients.
• Review medical history and social environment to inform care.
• Bridge care gaps in one visit.
