Hearts For Dementia LLC in Phoenix, AZ is seeking a committed Personal Care Attendant/Caregiver to join our growing team. This per diem role involves supporting clients at home in the East Valley and surrounding areas, helping with daily living activities, safety, and comfort.

You will assist with personal care, mobility, meals, medications, and light housekeeping while maintaining HIPAA and dignified care. Drivers license and reliable transportation are preferred.

Qualifications

• High school diploma or equivalent; caregiving certification preferred.
• Driver's license required.
• Minimum 5 years of caregiving experience is a plus.
• Knowledge of basic caregiving, personal hygiene and safety practices.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate, patient and empathetic demeanor.
• Reliable transportation preferred.
• REQUIRED TRAINING: CPR and First Aid is a plus; Direct Care Worker (DCW) certificate; Fingerprint Clearance Card.

Responsibilities

• Assist with personal care activities (bathing, grooming, dressing, toileting).
• Help with mobility and transfers (bed/vehicle).
• Provide companionship and emotional support.
• Assist with meal preparation and nutrition.
• Medication assistance and monitoring health conditions.
• Light housekeeping and safety of living environment.
• Document changes in clients' conditions to healthcare team.
• Maintain confidentiality and HIPAA compliance.
