Responsibilities

• Assists the physician and health care team in addressing social and emotional needs.
• Participates in interdisciplinary assessment and plan of care development.
• Develops and evaluates the plan of care with patient, family, and caregivers.
• Completes documentation addressing treatment, progress, and response to interventions.
