Ability to maintain yearly training, including Mandatory Reporter, HIPAA, OSHA.

Responsibilities

Provide direct services to Consumers, including varied hours (nights, weekends, holidays).
Assist Consumers with daily living skills and behavior management.
Support Consumers in achieving Individual Support Plan goals and attend appointments as needed.
Document HCBS outcomes each shift and maintain accurate records and timesheets.
Participate in interdisciplinary team meetings and communicate with families and staff.
Identify safety concerns and follow procedures to manage crises.
Maintain confidentiality and proper release of information.
Operate office equipment and coordinate with supervisor on incidents.
Support consumer empowerment and community inclusion.
