Job summary

PruittHealth Hospice is seeking a Flex RN Case Manager to support admissions, peaceful path visits, and routine visits in the Georgia hospice program. This role involves coordinating patient goals with the medical team and visiting patients in homes or care facilities.

Qualifications

• Current, active, and unrestricted RN or LPN licensure in the state of practice.
• Minimum of three years of nursing experience.
• Palliative care, hospice, or home health care experience preferred.
• CHPN certification preferred.

Responsibilities

• Take and process palliative care referrals.
• Support the Palliative Care NP and Medical Director.
• Schedule visits to support goals of care as ordered by MD.
• Coordinate and communicate patient care goals among providers.
• Contact families to present the palliative program and obtain consents.
• Document assessments and changes accurately and timely.
• Inform team of admissions and discharges.
• Coordinate referrals using the referral process.
• Serve as liaison for medical directors and NPs to improve effectiveness.
• Address billing holds and errors by running billing reports.
