Job summary

Aspire Home Healthcare of Ohio is seeking a skilled Physical Therapist (PT) to join our team for home health services in the Dublin area. You will evaluate, plan, and implement individualized treatment programs and coordinate with the broader rehabilitation team to help patients regain independence.

We offer flexible scheduling, competitive per-visit rates starting at $75, mileage reimbursement, and growth opportunities across states.

Qualifications

• Active Physical Therapist (PT) license in OH.
• Minimum 1 year of PT experience preferred.
• Current TB test (within 12 months) and CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and treat home health patients with individualized plans.
• Coordinate care with PTAs, HHAs, nurses and other clinicians.
• Document progress and treatment notes accurately and on time.
• Educate patients and families on exercises and use of adaptive equipment.
• Assess home environments and recommend safety modifications.
