Job summary

Concierge Home Care is seeking a Home Health Physical Therapist (PT) to deliver 1:1 care in patients' homes in Indian River County, FL. The role offers a flexible schedule and autonomy with the opportunity to increase earnings based on productivity.

You will evaluate patients, create individualized plans, provide therapeutic exercises, and collaborate with physicians while using AI-assisted charting and WellSky/Kinnser EMR to streamline documentation.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home Health PT – Flexible Schedule

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs).
• Complete documentation using streamlined technology.
