Job summary

Qualifications

• Active Florida OT license required.
• CPR/BLS certification required (Healthcare Providers).
• Proof of professional liability insurance required.
• Valid W-9 (we provide blank W-9 if needed).
• Reliable transportation to serve clients across Florida.

Responsibilities

• Perform home health OT evaluations and OASIS assessments.
• Develop individualized ADL/IADL-focused treatment plans.
• Provide interventions including adaptive equipment training and home modifications.
• Complete re-evaluations, recertifications, and discharge documentation.
• Educate patients and caregivers on safety and energy conservation.
• Maintain compliant documentation per Medicare/Medicaid guidelines.
