Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Pottstown, PA. The DSP provides hands-on, person-centered support to individuals with IDD in home and community settings, promoting independence and dignity.

Responsibilities include assisting with daily living activities, meal prep, mobility, and medication support, while implementing behavior plans and maintaining documentation. Flexibility and reliable transportation are valued assets.

Qualifications

High school diploma or equivalent.
Experience with IDD or autism is preferred.
Flexible schedule and willingness to travel.
English proficiency required (spoken and written).
Valid driver's license may be required based on assignment.

Responsibilities

Provide hands-on, person-centered support to individuals with IDD in home and community settings.
Assist with daily living activities and personal care.
Support meal preparation, nutrition, mobility, and medication assistance as authorized.
Implement behavior support strategies under supervision.
Monitor health, safety, and emotional well-being; promote independence and community participation.
Maintain accurate and timely documentation and participate in trainings and team meetings.
