Job summary

Vital Healthcare Solutions seeks a flexible Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide behavioral health medication management for residents in skilled nursing and residential settings. The role involves travel up to one hour from home, covering multiple facilities with a Monday–Friday schedule and a full benefits package.

Qualifications

• Active PMHNP certification and applicable state licensure.
• Experience in psychiatry or behavioral health; geriatric population experience preferred.

Responsibilities

• Provide psychiatric evaluations, medication management, and ongoing behavioral health care to residents in long-term care facilities.
• Assess, diagnose, and treat mental health conditions in the geriatric population.
• Develop and adjust individualized treatment plans based on assessments and patient response.
• Monitor medication effectiveness and side effects, and overall treatment outcomes.
• Collaborate with facility staff, physicians, and families to support quality, patient-centered care.
• Maintain accurate clinical documentation and comply with regulatory standards.
• Travel to assigned facilities as needed and adapt to changing schedules.
