Job summary

LiveWell Partners’ Apollo Medical Home Health unit seeks a compassionate Float Registered Nurse to join our home-based care team. As a Float RN, you will provide patient-centered nursing care in patients' homes, delivering flexible coverage across our territory.

You will collaborate with families, physicians, and care teams, perform assessments, educate patients on disease management, and maintain accurate EMR records. A supportive culture and opportunities for professional growth are offered.

Qualifications

• Minimum of 2 years of nursing experience, preferably in home health or acute/post-acute care.
• Active State RN licensure is required.
• Proof of current CPR certification and valid driver's license.

Responsibilities

• Provide high-quality, patient-centered care in patients' homes, ensuring outcomes and patient experience scores.
• Collaborates with patient, family/caregiver(s), physicians, case manager, care team members, and community resources.
• Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
• Conduct thorough patient assessments, perform skilled care, and educate patients and families on disease management and self-care.
• Maintain accurate, timely, and detailed records of patient care in the EMR system, including progress notes.
• Participate in on-call rotations.
• Develop and maintain positive relationships with patients, families, and caregivers, while promoting a culture of respect, empathy, and compassion.
