The position involves clinic and event coverage about one day per week, a shared call schedule, and collaboration with a diverse care team to holistically improve patient health. 1+ year APP experience is required and experience in FQHC/community health centers is valued.

Qualifications

• 1+ year APP experience required.
• Experience working in a FQHC or Community Health Center would be of great benefit to the practice.

Responsibilities

• Treating family medicine, pediatrics, and adults with complex issues.
• Co-manage patients with a diverse care team including RN, Counselors, and Care Management.
• Review quality and outcome metrics monthly with the healthcare team.
• Collaborate with local civic and faith-based leaders on community education programs.
• Flexible scheduling with clinic and event coverage averaging about 1 day per week.
