Job summary

PruittHealth Hospice is seeking a Flex RN Case Manager to support admissions, peaceful path visits, and routine visits in home or care facility settings. You will coordinate care with the Palliative Care NP and Medical Director, scheduling visits to meet goals of care as ordered by the physician.

Candidates should have 3+ years of nursing experience with RN or LPN licensure in the state. Palliative/hospice or home health experience is preferred, CHPN certification is a plus.

Qualifications

• Current, active RN or LPN licensure in the state of practice.
• Minimum of three years of nursing experience.
• Palliative care, hospice, or home health experience preferred.
• CHPN certification preferred.

Responsibilities

• Take and process palliative care referrals.
• Support the Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to align with goals of care as ordered by MD.
• Coordinate and communicate patient care goals between providers.
• Obtain consents and assess interdisciplinary needs.
• Document accurately and timely per policy.
• Inform team of new admissions and discharges.
• Coordinate referrals and serve as liaison with physicians.

KEY RESPONSIBILITIES:

· Responsible for taking and processing Palliative Care referrals.

· Provide support to the Palliative Care Nurse Practitioner and Medical Director.

· Will schedule visits to support goals of care as ordered by MD.

· Coordinate and communicate patient care goals between all providers.

· Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.

· Ensure communication of assessment and reassessment findings to attending physician.

· Complete all documentation accurately, timely, and in accordance with company policy and procedure.

· Inform palliative team of all new admissions and discharges.

· Coordinate referrals utilizing the referral process.

· Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.

· Run billing report and address validation holds/ billing errors

Qualifications

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:

· Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN)licensure in the state of practice

· A minimum of three (3) years of nursing experience

· Palliative care, hospice, or home health care experience is preferred

· CHPN (Certified Hospice and Palliative Nurse) is preferred
