Job summary

Christian Living Communities seeks a Physical Therapist to provide flexible, PRN clinical services for residents across skilled nursing and assisted living facilities in the Denver area. You will assess, plan, implement, and monitor rehabilitative plans, collaborating with physicians and care teams to support recovery and independence.

Requirements include graduation from an accredited therapy program, a valid PT license, CPR certification, and 2–3 years of related experience.

Qualifications

  • 2–3 years of related experience, or an equivalent combination of education and experience.
  • Current Physical Therapist state license; CPR certification.
  • Excellent verbal and written communication, active listening, and social perceptiveness.

Responsibilities

  • Restores residents’ function and relieves pain by planning and administering physical therapy.
  • Provides personalized care by assessing evaluations and test results.
  • Develops a treatment plan with goals and collaboration with staff and caregivers.
  • Determines physical therapy plans in consultation with physicians or by prescription.
  • Implements the treatment plan on a consistent schedule and adjusts as needed.
  • Understands residents' goals, preferences, and comfort level to build trust.
  • Helps residents complete their plan with manual exercises, massage, and equipment use.
  • Evaluates mobility and strength, setting attainable goals per resident ability.
  • Records progress and modifies treatment plans as needed.
  • Evaluates prosthetic/orthotic devices and suggests adjustments.
  • Documents care services per approved procedures.
  • Communicates progress with residents, families, and care teams clearly.
  • Develops post-treatment transition plans and coordinates in-home services.
  • Collaborates with team to foster an inclusive, supportive culture.

Job description

On a flexible, as-needed basis, provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include skilled nursing, assisted living, and/or rehabilitation services.

Rehabilitation Flex Team Members are exempt from a multiple-shift requirement, and are encouraged to cover needs at multiple sites as they are able.

Assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and support recovery from injury or illness. This role goes beyond clinical treatment - it's about building genuine, trust-based relationships with residents and their families, and contributing to a team culture that values the unique strengths each person brings, both on our staff and among the residents we serve.

  • Restores residents’ function, alleviates pain, and prevents further limitations by planning and administering medically prescribed physical therapy.
  • Provides quality, personalized care by assessing and interpreting evaluations and test results.
  • Develops a Plan of Treatment for each resident based on initial and ongoing evaluations, including short- and long-term goals, collaboration with other staff members, and appropriate resident and caregiver involvement.
  • Determines physical therapy treatment plans in consultation with physicians or by prescription.
  • Implements the Plan of Treatment on a consistent schedule, adjusting the plan as needed based on resident progress.
  • Takes time to understand each resident's goals, preferences, and comfort level, fostering trust throughout the treatment process.
  • Helps residents accomplish their treatment plan by administering manual exercises, providing massage therapy, and using equipment as necessary.
  • Evaluates mobility, strength, and current level of function, setting goals that are scaled to each resident's abilities and realistically attainable.

Basic Qualifications & Experience
  • Education: Graduate of an accredited therapy program.
  • Experience: 2–3 years of related experience, or an equivalent combination of education and experience.
  • Licenses/Certifications: Current Physical Therapist state license; CPR certification.

Working Conditions / Physical Requirements
  • Physical stamina to lift, transfer, and assist residents through physical activities.
  • Requires frequent lifting and carrying of items weighing up to 50 lbs. unassisted, including assisting residents when needed.
  • Must be able to communicate clearly with residents in person and over the phone.
  • Visual acuity adequate to perform job duties, including visual examination of residents and reading information from printed sources and computer screens.
