What to Expect (Job Responsibilities):
- Conduct comprehensive pre-admission screenings (initial and reassessments) in various settings using designated instruments
- Manage and monitor caseloads, schedule interviews, and document assessments within application timeframes
- Gather and review medical and relevant records to support eligibility determinations
- Collaborate with other state agencies to ensure comprehensive service delivery
- Provide applicants with outcomes of assessments and appropriate community resources
What is Required (Qualifications):
- Minimum of 3 years of experience in social service case management or related fields; or an accredited nursing degree with an active Arizona RN license; or a Bachelor's or Master's degree in a related field with 2 years of experience
- Knowledge of medical, functional, and psycho-social issues affecting the elderly and disabled populations
- Ability to communicate effectively in both oral and written forms
- Valid Arizona Driver License
- Fluency in Navajo is required for specific offices
How to Stand Out (Preferred Qualifications):
- Bilingual in Spanish, although not required
- Experience working with community resources and crisis intervention methods
- Familiarity with ALTCS policies and procedures
- Previous experience in health care delivery systems
- Strong analytical and organizational skills
