Brookside is currently hiring for a Fitness in the City (FITC) Case Manager to join our Nutrition department. The goal of the FITC program is to prevent and manage overweight/obesity in the pediatric patient population. FITC is grant-funded through Children's Hospital and Kohl's Cares. This work is based on-site at Brookside Community Health Center with some flexibility for remote work. Come join our diverse and collaborative team in this rewarding role!
Job Responsibilities:
*Work with care teams to identify and enroll eligible patients into FITC
*Meet with families and youth to assess level of engagement, screen for referral needs, conduct nutrition/fitness surveys, and provide food resources and activity referrals.
*Collaborate with stakeholders to plan and implement healthy lifestyle programming to address childhood obesity, including physical activity programming and nutritional instruction.
*Provide culturally sensitive patient-centered care to patients of all races, ethnicities, backgrounds and income levels
*Collaborate with dietitian to refer patients to nutrition counseling and coordinate patient goals
*Track behavioral changes through surveys and compiling data
*Make post-visit follow-up appointments and calls to assess progress
*Meet with and maintain working relationships with 11 other Fitness in the City Community Health Centers through cross collaboration with programs and resources
*Participate in performance improvement/quality assurance activities.
*Participate in department meetings, health center meetings, and FITC meetings.

Education
Bachelor's Degree required Preferred area of study: health management, childhood development, nutrition, public health or closely related field

Licenses and Credentials
Dietitian and Nutritionist [State License] preferred
Heartsaver CPR AED Certification [CPR] preferred
Registered Dietitian [RD] preferred
Registered Dietitican Nutritionist [RDN] preferred
Experience
Completed Internship
Knowledge, Skills & Abilities
Bilingual Spanish-speaking skills preferred
2 years of experience working with youth in a health care or educational setting preferred
Ability to provide counseling services to children and young adults
Knowledge of case management principles, skills and methods
Excellent written and oral communication skills.
Strong tech literacy
Strong teamwork and customer service skills
