Job summary

CVS Health is seeking a Field Social Worker to play a vital role in member-centered care planning for DSNP members. You will conduct home and community visits, complete assessments, set goals, identify barriers, and coordinate care with providers and care teams.

You will advocate for coverage under Medicaid and available resources while collaborating with hospital social workers and community partners to optimize care and reduce unnecessary stays.

Qualifications

• Must reside in Philadelphia, Pennsylvania or within commutable distance.
• Active LMSW or LCSW license in Pennsylvania.
• Valid driver’s license and reliable transportation.
• Ability to travel up to 50-75% within a designated geographic area.
• 2+ years of medical social work or case management.
• Proficient in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams).

Responsibilities

• Conduct field visits to the member's home, community locations, medical facilities, and other locations.
• Complete care management focused assessments.
• Set member-centered goals.
• Identify barriers and strategies to address them.
• Establish, monitor, and adapt interventions to achieve member goals.
• Collaborate and facilitate ongoing communication with providers, primary care physicians, Medical Directors, pharmacists, and care management staff.
• Advocate for members to ensure they receive care covered by Medicaid and available resources.
• Build relationships with hospital social workers and community resources to manage care and reduce lengths of stay.
• Identify and report potential quality improvement opportunities.
• Direct social work interventions including psychosocial assessments, in-home or facility assessments, documentation, and referrals.
