Horizon Case Management Corp seeks dedicated social work professionals to provide in-home and field-based patient assessments, care coordination, and education. You’ll collaborate with families, providers, and community programs to create individualized plans, navigate resources, and advocate for patient needs while maintaining confidentiality and compliance.

Applicants should be licensed in Michigan, have at least two years of experience in social work or case management, and be comfortable

Formación

• Current Social Work licensure in Michigan (LBSW, LMSW, or equivalent).
• Minimum 2 years of experience in social work or case management, preferably in a healthcare setting.
• Case Management Certification preferred, otherwise CCM Exam within 2 years of joining.

Responsabilidades

• Conduct in-home or field-based assessments to evaluate patients’ social, emotional, and healthcare needs.
• Collaborate with patients, family, and providers to develop and implement individualized care plans.
• Coordinate access to community resources and healthcare services for patients in the field.
• Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
• Work with physicians, nurses, and therapists to provide comprehensive, patient-centered care.
• Advocate for patients’ social and healthcare needs and assist navigating resources.
• Participate in ongoing professional development and share best practices with the team.
• Handle situations involving Adult/Child Protective Services and manage guardianship processes.
