Qualifications

• Bachelor's degree in nursing, social work, or related clinical field required.
• Master's degree in Social Work required.
• 1-2 years of experience in social service or health care setting preferred.
• Experience with vulnerable populations (Developmental Disabilities, HIV, Behavioral Health) preferred.
• LCSW license required.

Responsibilities

• Integrates physical health, behavioral health, and social/environmental factors into a person-centered plan of care.
• Promotes client-centered service planning aligned with member priorities and needs.
• Engages enrollees in planning through teams with expertise in community integration and health/wellness.
• Leverages community resources to ensure coordinated, culturally appropriate care.
• Performs field-based assessments and coordinates referrals across MedStar Family Choice programs.
