Job summary

21st Century Home Health Services is seeking an entry-level Field Registered Nurse in the Berkeley area to develop fundamental nursing skills in a home health setting. The role supports Case Management RN and delivers in-home care under supervision.

The position requires CA RN licensure, BLS, and a willingness to travel to patient homes. We offer comprehensive health benefits and a 401(k) plan with up to 4% match.

Qualifications

• Bachelor's degree in nursing required.
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) certification required.
• Valid CA Driver's License with acceptable driving record.
• One year of nursing experience; home health experience preferred.

Responsibilities

• Provide hands-on patient care within RN scope (PICC lines, wound care, NG/G/J tubes, drains, ostomy care, injections, IVs, and more).
• Document care timely within 24–48 hours per plan of care.
• Implement and support the care plan developed by Case Management RN.
• Educate patients and families about diagnosis and plan of care.
