In this RN Nurse Care Coordinator role, you will be an essential element of an Integrated Care Model by relaying pertinent information about member needs, advocating for the best possible care, and ensuring they have the right services to meet their needs. Our team comprises RNs, LVNs, and CACs dedicated to helping members achieve their health care goals. We work closely with nursing facilities where our members reside.

If you are located within Ellis and Johnson County, TX in the zip codes 75119, 76651, 75165, 76009, or 76031 and can travel locally, you will have the flexibility to work remotely while facing some challenging tasks. Travel in local areas is about 75% based on business needs.

This is a field-based position with a home-based office.

Primary Responsibilities
  1. Assess, plan, and implement care strategies tailored to each member, focusing on the least restrictive level of care.
  2. Identify and initiate referrals for member transition to community services and assist with social service programs, including financial, psychosocial, community, and state supportive services.
  3. Manage the care plan throughout the continuum as a single point of contact.
  4. Communicate with stakeholders to ensure timely, coordinated care and services.
  5. Advocate for members and families to ensure their needs and choices are respected and supported by the healthcare team. Visit Medicaid members in their homes and other settings such as community centers, hospitals, or providers’ offices.

Required Qualifications
  • Current, unrestricted RN license in Texas
  • At least 2 years experience in community health or healthcare roles
  • Intermediate computer skills including Outlook, Teams, and web applications
  • Reliable transportation and ability to travel locally to member homes and settings
  • Dedicated home workspace with PHI security
  • High-speed internet access
  • Valid US driver’s license
Preferred Qualifications
  • Over 1 year experience with long-term care, Medicaid, or Medicare
  • Proficiency in creating and managing documents and emails
  • Knowledge of federal and state regulations regarding care settings
  • Reside within commuting distance of assigned duties
  • Adherence to telecommuting policies

The salary range is $28.61 to $56.06 per hour, based on full-time employment, experience, education, and location. We offer a comprehensive benefits package, incentives, and stock options.
